Per California Probate Code § 4675, an Advance Healthcare Directive is completely legally invalid unless a state-designated Long-Term Care Ombudsman is physically present in the room to witness the execution alongside the notary public. Strict 5-Year ID Compliance (Civil Code § 1185):

Urgency does not alter consumer-protection mandates. Under California Civil Code § 1185, every principal signer must display an original, physical government-issued photo ID that is current or was issued within the past 5 years.
